Technology enabled care platform
Description:
A telecare system that provides regular monitoring of heart failure patients in their homes. Patients upload regular recordings of blood pressure, heart rate, blood oxygen levels and weight, using a tablet device.
Barriers:
Failures with equipment and/or connection issues. Patients putting in inaccurate readings.
Enablers:
Patients provide readings from home, avoiding travel to a clinic. Technical support is offered by the telecare developers. Patients without internet access are given a SIM card. Nurses follow-up on readings that are outside the patient's norm.
Outcomes for patient:
Reduced in-person clinic visits. Regular monitoring of readings by a specialist nurse. Care is escalated promptly for unwell patients.
